Canine Coast Pack Walks Waiver

In order to participate in our pack walks, workshops, services & events, we need to have a release of liability on file for you. Please fill out & submit the form below. Thank you!

I understand that there are certain risks involved in activities at which dogs and humans are present. I agree to not hold Sam Simon Charitable Foundation, its volunteers, board members or other representatives liable for any incidents which may occur at events, workshops, pack walks, and interactions in which Sam Simon Charitable Foundation participates or hosts. I agree to do my best to prevent any potential incidents by managing my dog safely on leash and under my control at all times, and to keep them from approaching or meeting other dogs. I also agree that should my dog need special equipment, like a muzzle, to keep them safe, that I will accommodate their needs and prioritize safety above all else.(Required)
Checking the items below means you also agree to follow our pack walk, workshop and event rules, including the following.(Required)
I agree to maintain at least 10 feet between my dog & others(Required)
I agree to have only one dog per adult handler at events(Required)
I agree to absolutely no dog intros(Required)
If I see a dog who needs more space, I will create more distance for them(Required)
